Tammy Cho

Practice Nurse

Tammy Cho, RN, BSN, has been with Vancouver Coastal Health for over 20 years. She began her career on the Neurosciences and Neurosurgery unit, expanding her expertise to the Stroke Unit, Seizure Investigation Unit, Step Down, and Neuro ICU. In 2008, she transitioned into critical care in the Vancouver General Hospital ICU and later served as a Patient Flow Supervisor at VGH, where she received the VCH People’s First Award for her leadership. For the past decade, Tammy has worked as a Chronic Disease Care Coordinator, supporting patients, families, and interdisciplinary teams through education and self-management support strategies in clinic, GP office, public library, on Fairchild TV, and with the Richmond Home and Community Care and Primary Care Network team. In 2025, Tammy joined the Northshore Primary Care Network bringing her passion with empowering patients to optimize their health and chronic disease management. Outside of work, Tammy enjoys yoga and nature walks and is currently pursuing her black belt in Taekwondo alongside her husband and two daughters, aged 16 and 11.
